{"id":271489,"date":"2024-08-09T20:03:34","date_gmt":"2024-08-10T00:03:34","guid":{"rendered":"https:\/\/policyoptions.irpp.org\/issues\/covid-analyse-eaux-usees\/"},"modified":"2025-10-08T00:09:41","modified_gmt":"2025-10-08T04:09:41","slug":"covid-analyse-eaux-usees","status":"publish","type":"issues","link":"https:\/\/policyoptions.irpp.org\/fr\/2024\/08\/covid-analyse-eaux-usees\/","title":{"rendered":"R\u00e9duire l\u2019analyse des eaux us\u00e9es p\u00e9nalise les plus vuln\u00e9rables et nous met tous \u00e0 risque"},"content":{"rendered":"<p style=\"text-align: right;\"><em>(English version available <a href=\"https:\/\/policyoptions.irpp.org\/magazines\/august-2024\/wastewater-testing-cutback\/\">here<\/a>)<\/em><\/p>\n<p><span data-contrast=\"auto\">Pendant la pand\u00e9mie de COVID-19, la surveillance des eaux us\u00e9es a rev\u00eatu une importance scientifique et politique cruciale, tout en fournissant au public des donn\u00e9es claires et facilement interpr\u00e9tables sur cette menace \u00e0 laquelle nous \u00e9tions tous confront\u00e9s.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-ccp-props=\"{}\">\u00a0<\/span><span data-contrast=\"auto\">La d\u00e9cision de <\/span><a href=\"https:\/\/ici.radio-canada.ca\/nouvelle\/2092950\/surveillance-indice-precoce-coronavirus-sante-publique\"><span data-contrast=\"none\">r\u00e9duire consid\u00e9rablement<\/span><\/a><span data-contrast=\"auto\"> ce programme en Ontario, un mod\u00e8le au pays, soul\u00e8ve d\u2019importantes questions sur l\u2019avenir de la surveillance en sant\u00e9 publique.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h3><b><span data-contrast=\"auto\">Complet, rapide et peu co\u00fbteux<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h3>\n<p><span data-contrast=\"auto\">Pourquoi les eaux us\u00e9es \u00e9taient-elles si importantes pour la surveillance des maladies infectieuses? Les progr\u00e8s r\u00e9alis\u00e9s dans la d\u00e9tection des agents pathog\u00e8nes en font un tierc\u00e9 rare du point de vue de la sant\u00e9 publique\u202f: exhaustivit\u00e9, rapidit\u00e9 et faible co\u00fbt.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Tout d\u2019abord, les maladies suffisamment graves pour \u00eatre port\u00e9es \u00e0 l\u2019attention des m\u00e9decins par l\u2019interm\u00e9diaire des patients ne repr\u00e9sentent presque toujours que la pointe de l\u2019iceberg\u202f: la surveillance bas\u00e9e sur des tests effectu\u00e9s sur des individus, bien que tr\u00e8s utile, est en effet limit\u00e9e et tr\u00e8s biais\u00e9e. En revanche, la surveillance des eaux us\u00e9es permet de savoir ce qui se passe dans tous les segments de la population, et souvent avec une pr\u00e9cision suffisante pour fournir des conseils sur mesure aux communaut\u00e9s vuln\u00e9rables ou pour les environnements \u00e0 haut risque (comme les centres de soins de longue dur\u00e9e).\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Deuxi\u00e8mement, en pr\u00e9sence d\u2019une maladie \u00e9mergence, cette surveillance fournit des informations <\/span><a href=\"https:\/\/www.frontiersin.org\/journals\/public-health\/articles\/10.3389\/fpubh.2023.1261165\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">suffisamment t\u00f4t<\/span><\/a><span data-contrast=\"auto\"> dans une vague pour permettre aux autorit\u00e9s gouvernementales et au public de <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2468042723000891\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">r\u00e9agir de fa\u00e7on appropri\u00e9e<\/span><\/a><span data-contrast=\"auto\">. Par exemple, les eaux us\u00e9es ont d\u00e9tect\u00e9 la variole du singe d\u00e8s qu\u2019<\/span><a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/73\/wr\/mm7302a3.htm\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">une seule personne par million<\/span><\/a><span data-contrast=\"auto\"> \u00e9tait infect\u00e9e. Cela a permis de profiter d\u2019un d\u00e9lai de plusieurs semaines avant que le risque pour la population g\u00e9n\u00e9rale ne devienne significatif. Les individus ont ainsi pu prendre des pr\u00e9cautions (comme mettre leurs vaccins \u00e0 jour), et les syst\u00e8mes de sant\u00e9 ont pu se pr\u00e9parer \u00e0 une hausse attendue des cas de maladie grave.\u00a0\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Troisi\u00e8mement, la surveillance des eaux us\u00e9es est incroyablement rentable. Bien que les estimations soient un peu vagues \u2013 ce qui est un enjeu de transparence, en plus des pr\u00e9occupations pour la sant\u00e9 publique \u2013 une \u00e9valuation r\u00e9cente a chiffr\u00e9 le budget de fonctionnement \u00e0 <\/span><a href=\"https:\/\/www.theglobeandmail.com\/canada\/article-ontario-health-minister-defends-end-of-covid-wastewater-program\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">moins de 15\u202fmillions\u202f$<\/span><\/a><span data-contrast=\"auto\"> pour l\u2019Ontario. C\u2019est l\u2019\u00e9quivalent de 1\u202f$ par personne, par ann\u00e9e, ce qui est plus abordable que la plupart des formes de surveillance.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">L\u2019analyse des eaux us\u00e9es fait appel \u00e0 une technologie remarquablement souple dans ses applications potentielles, non seulement pour la COVID-19, mais aussi pour de <\/span><a href=\"https:\/\/irpp-my.sharepoint.com\/:w:\/g\/personal\/lperreaux_irpp_org\/EUrwxBvj9pVBnoNOoc1nff0BajoZpv0nbvEaqWl__wSpVw\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">nombreux autres virus<\/span><\/a><span data-contrast=\"auto\">, dont le VRS, la <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMc2213882\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">variole du singe<\/span><\/a><span data-contrast=\"auto\">, ainsi que les grippes <\/span><a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2024.05.10.24307179v1\"><span data-contrast=\"none\">aviaire et humaine<\/span><\/a><span data-contrast=\"auto\">. Elle est \u00e9galement utile pour suivre et r\u00e9agir \u00e0 la <\/span><a href=\"https:\/\/www.scientificamerican.com\/article\/the-opioid-crisis-is-now-being-tracked-with-wastewater\/\"><span data-contrast=\"none\">crise des opio\u00efdes<\/span><\/a><span data-contrast=\"auto\"> et pour \u00e9valuer l\u2019exposition de la population <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9927796\/\"><span data-contrast=\"none\">aux toxines et aux hormones<\/span><\/a><span data-contrast=\"auto\">, en plus d\u2019\u00eatre prometteuse pour surveiller la pand\u00e9mie de plus en plus dangereuse de r\u00e9sistance aux antimicrobiens.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Nous ne savons pas de quoi aura l\u2019air une future \u00ab<\/span><span data-contrast=\"auto\">\u2009<\/span><a href=\"https:\/\/cepi.net\/disease-x-what-it-and-what-it-not\"><span data-contrast=\"none\">maladie X<\/span><\/a><span data-contrast=\"auto\">\u2009<\/span><span data-contrast=\"auto\">\u00bb, mais la surveillance des eaux us\u00e9es constituera tr\u00e8s certainement un \u00e9l\u00e9ment essentiel d\u2019un <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41591-022-01940-x\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">syst\u00e8me d\u2019alerte pr\u00e9coce et souple<\/span><\/a><span data-contrast=\"auto\">, destin\u00e9 \u00e0 prot\u00e9ger les Canadiens.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h3><b><span data-contrast=\"auto\">L\u2019affaiblissement d\u2019un outil essentiel<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/h3>\n<p><span data-contrast=\"auto\">En 2023, plus de 100\u202fsites d\u2019analyse des eaux us\u00e9es fonctionnaient en Ontario, couvrant ainsi 75\u202f% de la population. L\u2019Agence de la sant\u00e9 publique du Canada a repris ce programme la semaine derni\u00e8re, en ne conservant que quatre sites autour de la ville de Toronto, ce qui couvre 20\u202f% de la population de la province. Quatre autres sites doivent \u00eatre op\u00e9rationnels d\u2019ici l\u2019hiver.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Cette importante perte de capacit\u00e9 de surveillance a d\u2019importantes implications en mati\u00e8re d\u2019\u00e9quit\u00e9, puisqu\u2019elle entrave la capacit\u00e9 de d\u00e9tection de la propagation de la maladie chez les Canadiens vivant en milieu rural, dans les petites communaut\u00e9s et dans le nord de la province, toutes des r\u00e9gions o\u00f9 l\u2019acc\u00e8s aux soins hospitaliers est d\u00e9j\u00e0 limit\u00e9.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">La vague de COVID-19 <\/span><a href=\"https:\/\/www.ledevoir.com\/societe\/sante\/817610\/covid-19-estivale-epargne-pas-canada\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">qui circule cet \u00e9t\u00e9<\/span><\/a><span data-contrast=\"auto\"> met aussi en relief d\u2019autres probl\u00e8mes d\u2019\u00e9quit\u00e9 qui se posent lorsque les capacit\u00e9s de surveillance diminuent. En l\u2019absence de mesures de sant\u00e9 publique, la prise de d\u00e9cision concernant la protection contre la COVID a \u00e9t\u00e9 principalement transf\u00e9r\u00e9e \u00e0 l\u2019individu, des strat\u00e9gies comme le port du masque ou la vaccination restant les seules options.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h3><b><span data-contrast=\"auto\">Les donn\u00e9es informent les d\u00e9cisions pour les individus aussi<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h3>\n<p><span data-contrast=\"auto\">Comment les individus peuvent-ils prendre des d\u00e9cisions sans les informations n\u00e9cessaires pour \u00e9valuer les risques auxquels ils font face? L\u2019\u00e9volution de cette vague estivale est un parfait exemple des d\u00e9fis qui se posent, comme le montre le r\u00e9sultat des analyses finales des eaux us\u00e9es affich\u00e9es la semaine derni\u00e8re et qui <\/span><a href=\"https:\/\/www.publichealthontario.ca\/en\/Data-and-Analysis\/Infectious-Disease\/COVID-19-Data-Surveillance\/Wastewater\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">laissent planer le suspense<\/span><\/a><span data-contrast=\"auto\"> sur la suite des choses.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Par exemple, les vaccins mis \u00e0 jour ne seront disponibles qu\u2019\u00e0 la fin de l\u2019automne, puisque les entreprises se concentrent sur la production d\u2019une version qui cible le variant\u202fKP.2. Or, de nombreuses personnes \u00e0 haut risque ont re\u00e7u leur derni\u00e8re dose de vaccin il y a plus de six mois et devront attendre encore trois mois avec une protection d\u00e9croissante, pendant que la propagation du virus risque de s\u2019accro\u00eetre autour d\u2019elles.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Ces personnes devraient-elles se faire vacciner maintenant avec la version du vaccin de l\u2019an dernier, ou attendre une version \u00e0 jour qui correspond mieux aux variants qui courent pr\u00e9sentement? De tels questionnements devraient pouvoir amener les patients et ceux qui les soignent \u00e0 prendre des d\u00e9cisions guid\u00e9es crucialement par les donn\u00e9es face au risque. Le probl\u00e8me est que les informations d\u00e9coulant de la surveillance, qui guideraient cette \u00e9valuation, ne sont plus disponibles pour beaucoup de gens.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">En ce mois d\u2019ao\u00fbt \u00e9touffant, la COVID-19 est, \u00e0 juste titre et heureusement, bien loin des pens\u00e9es de la plupart des Canadiens. Mais r\u00e9duire consid\u00e9rablement un programme qui a d\u00e9montr\u00e9 sa valeur fondamentale, et renforc\u00e9 l\u2019\u00e9quit\u00e9 et notre capacit\u00e9 \u00e0 soigner les personnes les plus vuln\u00e9rables, ne rencontre pas les attentes minimales envers les pratiques de sant\u00e9 publique.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">L\u2019absence de ces informations rend les d\u00e9cisions en mati\u00e8re de soins plus difficiles pour les plus vuln\u00e9rables d\u2019entre nous et pour ceux qui vivent dans les r\u00e9gions d\u00e9j\u00e0 mal desservies de notre pays. En plus, elle fait en sorte que nous serons tous moins bien pr\u00e9par\u00e9s \u00e0 faire face aux futures \u00e9pid\u00e9mies et pand\u00e9mies.<\/span><span
