La question a \u00e9t\u00e9 soulev\u00e9e <\/span><a href=\"https:\/\/www.lapresse.ca\/actualites\/covid-19\/2021-04-03\/sante-publique-ou-sante-politique.php\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">\u00e0 de maintes reprises<\/span><\/a><span data-contrast=\"auto\"> ces derni\u00e8res ann\u00e9es au Qu\u00e9bec.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\"><a href=\"https:\/\/policyoptions.irpp.org\/fr\/magazines\/octobre-2023\/covid19-science-opinion-publique\/\">Nos recherches sur la pand\u00e9mie<\/a> montrent que l\u2019influence des experts est conditionnelle au soutien du public, et que cette influence ne s\u2019exerce pas dans un contexte purement rationnel o<\/span><span data-contrast=\"auto\">\u00f9<\/span><span data-contrast=\"auto\"> aucune autre force n\u2019interviendrait. Concr\u00e8tement, cela pose la question des orientations politiques de la communication scientifique.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Une comparaison que nous avons r\u00e9alis\u00e9e avec des coll\u00e8gues belges, su\u00e9dois et suisses indique qu\u2019au Qu\u00e9bec, les recommandations scientifiques transmises aux d\u00e9cideurs ont \u00e9t\u00e9, plus qu\u2019ailleurs, sous tutelle politique. En outre, les d\u00e9cideurs politiques ont conserv\u00e9 un contr\u00f4le \u00e9troit sur la communication des recommandations qui leur \u00e9tait faite par la sant\u00e9 publique, se traduisant par une transparence limit\u00e9e des informations scientifiques appuyant les d\u00e9cisions.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<h3><b><span data-contrast=\"auto\">Qu\u00e9bec\u202f: une sant\u00e9 publique subordonn\u00e9e au politique<\/span><\/b><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/h3>\n<p><span data-contrast=\"auto\">Au Qu\u00e9bec, <\/span><a href=\"https:\/\/link.springer.com\/article\/10.17269\/s41997-018-0080-3\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">la Sant\u00e9 publique est directement subordonn\u00e9e au minist\u00e8re de la Sant\u00e9 et des Services sociaux (MSSS)<\/span><\/a><span data-contrast=\"auto\"> de telle fa\u00e7on que les experts de l\u2019Institut national de sant\u00e9 publique du Qu\u00e9bec (INSPQ), ou m\u00eame le directeur national de la sant\u00e9 publique \u2013 qui est <\/span><a href=\"https:\/\/www.quebec.ca\/gouvernement\/ministere\/sante-services-sociaux\/organigramme\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">\u00e9galement sous-ministre adjoint<\/span><\/a><span data-contrast=\"auto\"> au MSSS \u2013, ne peuvent pas communiquer publiquement de mani\u00e8re autonome et transparente.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Dans d\u2019autres pays, une approche plus transparente et volontariste, centr\u00e9e autour d\u2019agences de sant\u00e9 publique, a permis d\u2019impliquer davantage le public et des acteurs externes \u00e0 la d\u00e9cision.\u00a0 Par exemple, en reconnaissant publiquement le manque de preuves scientifiques solides pour justifier la fermeture des \u00e9coles en d\u00e9but de pand\u00e9mie<\/span><span data-contrast=\"auto\">, <\/span><a href=\"https:\/\/cirst.uqam.ca\/blogue-du-cirst\/ecouter-la-science-dans-la-conception-des-politiques-publiques-de-lutte-contre-la-covid-19-le-cas-de-la-fermeture-des-ecoles-au-quebec-et-en-suede\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">l\u2019agence de sant\u00e9 publique su\u00e9doise a provoqu\u00e9 un important d\u00e9bat<\/span><\/a><span data-contrast=\"auto\">.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Au Qu\u00e9bec, la communication des recommandations de la sant\u00e9 publique ayant \u00e9t\u00e9 \u00e9troitement contr\u00f4l\u00e9e par les autorit\u00e9s politiques, les d\u00e9bats autour des preuves qui auraient justifi\u00e9 ou non une mesure sanitaire donn\u00e9e se sont faits plus rares.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Certaines recommandations d\u2019experts <\/span><a href=\"https:\/\/publications.msss.gouv.qc.ca\/msss\/document-002897\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">ont bien \u00e9t\u00e9 rendues publiques au Qu\u00e9bec<\/span><\/a><span data-contrast=\"auto\">. Toutefois, elles sont demeur\u00e9es tr\u00e8s sommaires, et <\/span><a href=\"https:\/\/ici.radio-canada.ca\/nouvelle\/1851196\/msss-revue-editeur-predateur-publication-scientifique\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">ont parfois justifi\u00e9 avec difficult\u00e9 des mesures importantes comme le couvre-feu<\/span><\/a><span data-contrast=\"auto\">.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">La <\/span><a href=\"https:\/\/plus.lapresse.ca\/screens\/e29bdf70-c56a-4c5c-9326-83d66b098d81%7C_0.html\"><span data-contrast=\"none\">subordination politique<\/span><\/a><span data-contrast=\"auto\"> a en effet eu pour cons\u00e9quence que les recommandations de la sant\u00e9 publique \u00e9taient pr\u00e9sent\u00e9es \u00e0 la <\/span><a href=\"https:\/\/lactualite.com\/politique\/les-48-heures-qui-ont-mene-au-reconfinement-du-quebec\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">cellule de crise dirig\u00e9e par le premier ministre<\/span><\/a><span data-contrast=\"auto\">, de sorte qu\u2019aucune personne ext\u00e9rieure \u00e0 cette cellule ne pouvait \u00eatre impliqu\u00e9e dans les d\u00e9lib\u00e9rations. Au Qu\u00e9bec, il n\u2019existait donc pas de s\u00e9paration nette entre les experts et les d\u00e9cideurs.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<h3><b><span data-contrast=\"auto\">Suisse\u202f: une \u00ab\u2009task force\u2009\u00bb ind\u00e9pendante<\/span><\/b><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/h3>\n<p><span data-contrast=\"auto\">\u00c0 l\u2019inverse, en Suisse, <\/span><a href=\"https:\/\/sciencetaskforce.ch\/fr\/page-daccueil\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">le groupe d\u2019experts formellement mandat\u00e9 par le gouvernement<\/span><\/a><span data-contrast=\"auto\"> pour le conseiller sur les mesures \u00e0 suivre durant la pand\u00e9mie n\u2019\u00e9tait pas issu d\u2019une agence gouvernementale ou d\u2019un minist\u00e8re. La \u00ab\u2009Swiss COVID-19 Science Task Force\u2009\u00bb (SN-STF), a \u00e9t\u00e9 cr\u00e9\u00e9e <\/span><a href=\"https:\/\/www.re-check.ch\/wordpress\/en\/science-pandemic-task-force\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">\u00e0 l\u2019initiative d\u2019experts et d\u2019universitaires<\/span><\/a><span data-contrast=\"auto\">.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Une distinction fondamentale avec le Qu\u00e9bec est que les recommandations du SN-STF ont \u00e9t\u00e9 formul\u00e9es \u00e0 l\u2019ext\u00e9rieur des lieux de d\u00e9cision et <\/span><a href=\"https:\/\/sciencetaskforce.ch\/fr\/policy-briefs-francais\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">rendues publiques de fa\u00e7on syst\u00e9matique<\/span><\/a><span data-contrast=\"auto\">, ce qui a eu pour cons\u00e9quence de faire ressortir certaines <\/span><a href=\"https:\/\/www.tdg.ch\/un-epidemiologiste-demissionne-du-groupe-de-travail-779948589977\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">dissensions entre experts et d\u00e9cideurs politiques<\/span><\/a><span data-contrast=\"auto\">, tout en favorisant des d\u00e9bats publics sur la gestion de la pand\u00e9mie.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">La r\u00e9sistance des d\u00e9cideurs politiques \u00e0 la transparence peut s\u2019expliquer. En d\u00e9mocratie, il appartient aux \u00e9lus de <\/span><a href=\"https:\/\/theconversation.com\/fonderie-horne-quel-role-occupent-les-preuves-scientifiques-dans-la-decision-politique-au-quebec-189738\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">d\u00e9cider en fonction de consid\u00e9rations multiples<\/span><\/a><span data-contrast=\"auto\">, au sein desquelles l\u2019avis des experts et les preuves scientifiques n\u2019en repr\u00e9sentent qu\u2019un seul aspect. De plus, lorsque les d\u00e9cideurs \u00e9lus choisissent l\u00e9gitimement de prendre une d\u00e9cision qui va \u00e0 l\u2019encontre de l\u2019avis des experts, <\/span><a href=\"https:\/\/ici.radio-canada.ca\/nouvelle\/1722826\/covid-isolement-symptomes-courriels-legault-arruda-drouin-montreal-quebec\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">ils se le font bien souvent reprocher.<\/span><\/a><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">En Belgique, un pays durement touch\u00e9 par la COVID-19, les d\u00e9cideurs ont d\u00fb composer avec un contexte politique f\u00e9d\u00e9ral complexe. Les avis des experts \u2013 qui sont communiqu\u00e9s plus librement qu\u2019au Qu\u00e9bec \u2013 se sont aussi parfois <\/span><a href=\"https:\/\/doi.org\/10.1007\/s11115-022-00629-6\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">inscrits en opposition aux d\u00e9cisions politiques<\/span><\/a><span data-contrast=\"auto\">, que les dirigeants ont pein\u00e9 \u00e0 justifier.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">On peut donc comprendre que dans le contexte d\u2019une crise comme celle de la COVID-19, les d\u00e9cideurs qu\u00e9b\u00e9cois aient voulu retenir toute information scientifique susceptible de contredire leurs d\u00e9cisions. \u00c9tait-ce l\u00e0 la meilleure approche? Nous pensons que non.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<h3><b><span data-contrast=\"auto\">Les cons\u00e9quences d\u2019une transparence limit\u00e9e<\/span><\/b><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/h3>\n<p><span data-contrast=\"auto\">D\u2019abord, la transparence limit\u00e9e au Qu\u00e9bec a pu contribuer au fatalisme du public. Au moment de rehausser la s\u00e9v\u00e9rit\u00e9 des mesures sanitaires, le contr\u00f4le serr\u00e9 de l\u2019information scientifique a permis aux d\u00e9cideurs de projeter vague apr\u00e8s vague une image unifi\u00e9e des preuves alarmistes, en d\u00e9pit de leur incertitude.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">\u00c0 l\u2019oppos\u00e9, une plus grande transparence aurait mis de l\u2019avant une plus grande diversit\u00e9 de preuves, certaines alarmistes, d\u2019autres moins, alimentant la r\u00e9flexion du public et les d\u00e9bats d\u2019experts quant \u00e0 la n\u00e9cessit\u00e9 de rehausser la s\u00e9v\u00e9rit\u00e9 des mesures.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">C\u2019est pr\u00e9cis\u00e9ment ce qui s\u2019est produit en Su\u00e8de. Encourag\u00e9 \u00e0 se former une opinion libre et \u00e9clair\u00e9e, le public su\u00e9dois semble \u00eatre moins tomb\u00e9 dans le fatalisme, notamment <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/34580225\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">en pla\u00e7ant la confiance envers les autres<\/span><\/a><span data-contrast=\"auto\"> devant un soutien \u00e0 des mesures plus s\u00e9v\u00e8res.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Au Qu\u00e9bec, face \u00e0 un ensemble de preuves communiqu\u00e9 strat\u00e9giquement par les d\u00e9cideurs et ne laissant que peu de place au choix, la population ne pouvait devenir qu\u2019indiff\u00e9rente sur le long terme. Cette indiff\u00e9rence <\/span><a href=\"https:\/\/theconversation.com\/decrochage-de-la-population-aux-mesures-sanitaires-une-sante-publique-plus-autonome-est-necessaire-176629\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">contribua \u00e0 r\u00e9duire son soutien aux mesures sanitaires<\/span><\/a><span data-contrast=\"auto\">, et avec elle l\u2019influence des experts.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Ensuite, le manque de transparence est susceptible d\u2019avoir contribu\u00e9 \u00e0 des surr\u00e9actions dommageables. Le contr\u00f4le de l\u2019information scientifique a accentu\u00e9 certains biais, dont les sentiments n\u00e9gatifs et de peur, qui ont sans doute \u00e9t\u00e9 utilis\u00e9s pour faire accepter des mesures s\u00e9v\u00e8res.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Plus de transparence aurait pu diminuer l\u2019influence de ces biais, puisque la pr\u00e9sence de preuves contradictoires aurait probablement favoris\u00e9 un raisonnement mieux inform\u00e9, et donc plus \u00e9quilibr\u00e9.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<h3><b><span data-contrast=\"auto\">Une mauvaise strat\u00e9gie \u00e0 long terme<\/span><\/b><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/h3>\n<p><span data-contrast=\"auto\">D\u2019un point de vue strat\u00e9gique, l\u2019utilisation des sentiments n\u00e9gatifs par les d\u00e9cideurs qu\u00e9b\u00e9cois, facilit\u00e9e par leur contr\u00f4le de l\u2019information scientifique, a bien fonctionn\u00e9. La preuve \u00e9tant le soutien \u00e9lev\u00e9 du public jusqu\u2019\u00e0 la cinqui\u00e8me vague.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Toutefois, \u00e0 partir de la cinqui\u00e8me vague, le public devenu indiff\u00e9rent <\/span><span data-contrast=\"auto\">\u2013 voire d\u00e9sensibilis\u00e9 \u00e0 la gestion par la peur \u2013<\/span><span data-contrast=\"auto\"> a fini par d\u00e9crocher. Les d\u00e9cideurs ajust\u00e8rent les mesures sanitaires en cons\u00e9quence en diminuant leur s\u00e9v\u00e9rit\u00e9 alors m\u00eame que les projections laissaient entrevoir d\u2019importantes difficult\u00e9s dans les h\u00f4pitaux.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Or, en temps de crise, la transparence au sujet des <\/span><span data-contrast=\"auto\">preuves<\/span><span data-contrast=\"auto\"> scientifiques qui <\/span><span data-contrast=\"auto\">informent<\/span><span data-contrast=\"auto\"> les <\/span><span data-contrast=\"auto\">d\u00e9cideurs<\/span><span data-contrast=\"auto\"> semble avoir produit des <\/span><span data-contrast=\"auto\">politiques<\/span><span data-contrast=\"auto\"> plus \u00e9quilibr\u00e9es.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Entre la Su\u00e8de, la Suisse et la Belgique, le Qu\u00e9bec occupe la seconde place en termes de s\u00e9v\u00e9rit\u00e9 des mesures sanitaires.
