I get hate mail. Some of it is comprehensive (\u201cI have yet to agree with any argument you have made about anything\u201d). Some of it is pretty frivolous (there are people who really don\u2019t like my glasses). Some seems ideologically motivated (\u201cM.A.G.A., Libtard!\u201d). And some of it is beautifully succinct (\u201cF*ck you, Timothy. Just f*ck you\u201d).<\/p>\n<p>But within all the noise and the ad hominem attacks, interesting themes have emerged. Over the years I\u2019ve noticed that no matter the topic, people let fly with many of the same complaints and insults. Whether I\u2019m writing about homeopathy, acupuncture, celebrity health nonsense, supplements, the wellness industry, genetic testing or unproven stem cell therapies, people seem to be angry about similar things.<\/p>\n<p>Not long ago I worked with my University of Alberta colleague <a href=\"https:\/\/www.ualberta.ca\/health-law-institute\/people\/alessandro-marcon\">Alessandro Marcon<\/a> to undertake a more systematic examination of the comments I receive. We picked an <a href=\"https:\/\/www.theglobeandmail.com\/opinion\/stop-those-naturopaths-who-spread-anti-vaxxer-myths\/article35444890\/\">article<\/a> I wrote for the <em>Globe and Mail<\/em> about vaccination policy and naturopaths. My piece highlighted the value of vaccination and the problems with the <a href=\"https:\/\/academic.oup.com\/jlb\/article-abstract\/4\/2\/229\/3871793\">antivaxx rhetoric<\/a> flowing from the naturopathy community. I got a lot of push-back, likely because it touched on two polarizing topics: vaccination safety and the efficacy of alternative health care practices. While the vast majority of Canadians support routine vaccination, there remain a vocal minority who are hardcore antivaxxers. And many others feel passionate about the value of alternative medicine.<\/p>\n<p>While not all of the comments were aggressive and hate-mail-ish (4.8 percent of the comments were removed by the <em>Globe<\/em> for not meeting its civility standard, so we likely missed some of the best stuff), we thought this analysis could serve to document central messages. These readers were motivated enough to write to the paper, so it is safe to assume they were highlighting their key concerns.<\/p>\n<p>So, what did we hear after looking at 330 comments made by 116 different individuals? The bottom line: Trust is a huge issue.<\/p>\n<p>Again and again, commenters raised the issue of a lack of trust \u2014 in researchers, physicians, pharmaceutical companies, government, the \u201ccorporate\u201d media and, of course, me. At the core of many of these claims was the role of the profit motive. Studies including <a href=\"https:\/\/www.liebertpub.com\/doi\/abs\/10.1089\/bio.2012.0029\">research<\/a> I did with my colleagues at the Health Law Institute have consistently <a href=\"https:\/\/journals.plos.org\/plosone\/article?id=10.1371\/journal.pone.0175643\">demonstrated<\/a> that funding from industry erodes, rightly or not, public trust in research \u2014 and this is particularly so in the context of vaccines. Indeed, a 2017 <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/27909947\">study<\/a> exploring the concerns of vaccination-hesitant parents found that \u201cperceptions of the profit motive generate distrust in the expert systems pertaining to vaccination.\u201d There has also been an erosion of trust in the medical profession. In the US, <a href=\"https:\/\/www.nytimes.com\/2018\/01\/23\/upshot\/do-you-trust-the-medical-profession.html\">for example<\/a>, only 34 percent have great confidence in medical leaders. In 1966 that number was 73 percent. That is a huge decline, one that makes room for alternative sources of scientific information to become more dominant.<\/p>\n<blockquote><p>Again and again, commenters raised the issue of a lack of trust \u2014 in researchers, physicians, pharmaceutical companies, government, the \u201ccorporate\u201d media and, of course, me.<\/p><\/blockquote>\n<p>Some of the trust concerns we heard were likely fuelled by evidence-free <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24586574\">conspiracy theories<\/a> that suggest the government is in cahoots with industry to sell vaccines they (allegedly) know are dangerous. Shockingly, a 2014 <a href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/1835348\">survey<\/a> found that 20 percent of the Americans believe this to be true and 36 percent are not sure. But we shouldn\u2019t forget that there have been many high-profile <em>real<\/em> events that the public can point to in order to justify a growing lack of trust.<\/p>\n<p>In just the past few months there have been <a href=\"https:\/\/www.nytimes.com\/2018\/10\/29\/health\/dr-piero-anversa-harvard-retraction.html\">stories<\/a> about an influential stem cell researcher falsifying data, leading to the retraction of dozens of studies in high-impact journals and the <a href=\"https:\/\/geneticliteracyproject.org\/2018\/10\/22\/calling-for-a-halt-to-heart-stem-cell-trial-based-on-controversial-research\/\">cancellation<\/a> of once promising clinical trials. It was also recently revealed that a well-known cancer researcher <a href=\"https:\/\/www.nytimes.com\/2018\/09\/08\/health\/jose-baselga-cancer-memorial-sloan-kettering.html\">failed<\/a> to disclose millions of dollars of industry funding. There are, of course, many other notorious examples of industry misconduct, including the <a href=\"https:\/\/www.nytimes.com\/2008\/04\/15\/business\/15cnd-vioxx.html\">Vioxx scandal<\/a> from just a few years ago that resulted in criminal charges and a billion-dollar fine. And it is well documented that industry involvement can impact the production and dissemination of biomedical knowledge \u2014 a reality that is arguably becoming more problematic as researchers face intensifying pressure to <a href=\"https:\/\/bmcmedethics.biomedcentral.com\/articles\/10.1186\/s12910-015-0064-2\">commercialize<\/a> their work.<\/p>\n<figure id=\"attachment_69773\" aria-describedby=\"caption-attachment-69773\" style=\"width: 624px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/policyoptions.irpp.org\/wp-content\/uploads\/2025\/08\/8235982.jpg\"><img fetchpriority=\"high\" decoding=\"async\" class=\"wp-image-69773\" src=\"https:\/\/policyoptions.irpp.org\/wp-content\/uploads\/2025\/08\/8235982.jpg\" alt=\"\" width=\"624\" height=\"557\" \/><\/a><figcaption id=\"caption-attachment-69773\" class=\"wp-caption-text\">AP Photo\/Daniel Hulshizer, file<\/figcaption><\/figure>\n<p>The comments attacking my <em>Globe<\/em> article also referenced \u2014 no surprise \u2014 a lack of trust in me. They said I\u2019m not qualified to comment, or that I\u2019m trying to help bury the true research, or that \u201cBig Pharma\u201d has purchased my opinion. Again, this kind of attack is a very common tactic both for those advocating for alternative practices and for those speaking against vaccines. It serves to discredit the messenger (me) and the facts I use to support my argument. And, unfortunately, research has shown that these kinds of personal attacks are effective. A 2018 <a href=\"https:\/\/journals.plos.org\/plosone\/article?id=10.1371\/journal.pone.0192025\">study<\/a> found that \u201cad hominem attacks may have the same degree of impact as attacks on the empirical basis of the science claims.\u201d<\/p>\n<p>(For the <a href=\"https:\/\/www.ualberta.ca\/health-law-institute\/people\/timothy-caulfield\">record<\/a>, I do not receive industry funding for my research or for the support of my students or research team. I do no paid consulting for industry or anyone else. I have no financial interest that requires disclosure in any company. I am not paid for any of the committee work I do. The show I host, <em><a href=\"https:\/\/www.statnews.com\/2018\/09\/28\/tim-caulfield-netflix-show-health-trends\/\">A User\u2019s Guide to Cheating Death<\/a><\/em>, was not funded by Big Pharma or the medical profession. And, despite frequent allegations to the contrary, no company has ever approached me with a request to alter my research or opinion. I work with a <a href=\"https:\/\/www.speakers.ca\">speakers\u2019 bureau<\/a>, a commercial <a href=\"https:\/\/www.penguinrandomhouse.com\/books\/417402\/is-gwyneth-paltrow-wrong-about-everything-by-timothy-caulfield\/9780143189039\/\">publisher<\/a> and a <a href=\"https:\/\/peacockalleytv.com\">TV production company<\/a>.)<\/p>\n<p>Those wishing to put forward a particular perspective \u2014\u00a0such as questioning the safety of vaccines \u2014 are obviously aware that leveraging the public\u2019s mistrust can have a big impact on public discourse. As noted in the conclusion of the 2018 <a href=\"https:\/\/journals.plos.org\/plosone\/article?id=10.1371\/journal.pone.0192025\">study<\/a>, \u201c91% of anti-vaccine websites explicitly claim that the biomedical field is rife with conflicts of interests and this communication tactic may play a part in the success of the anti-vaccine movement.\u201d<\/p>\n<p class=\"dropcap\">The reasons people turn to alternative therapies and away from <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5638294\/\">vaccination<\/a> are <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24598724\">complex<\/a> and vary from individual to individual. But a lack of trust in the conventional health care system and the research enterprise is increasingly a <a href=\"https:\/\/www.thestar.com\/opinion\/commentary\/2017\/11\/09\/a-lack-of-trust-not-of-science-behind-vaccine-resistance.html\">key<\/a> part of these social trends. Supporting policies that facilitate independent science and research integrity not only will lead to the production of good evidence, but will also make it easier to combat the spread of misinformation.<\/p>\n<p>The tone of these missives could certainly be improved, and very frequently they involve odd logic leaps or stuff that is simply factually inaccurate (there is still <em>no<\/em> evidence to support the claim that vaccination is linked to autism). But the consistency of the trust concern is telling. Getting hate mail usually isn\u2019t the best part of my day \u2014 though some of it is pretty darn funny and creative \u2014 but it can provide insight into what is driving the embrace and spread of health misinformation.<\/p>\n<p><span class=\"image-caption\">Photo:\u00a0Shutterstock\/By Rawpixel.com<\/span><\/p>\n<hr \/>\n<p><em>Do you have something to say about the article you just read?
