One of these is\u00a0the\u00a0Canadian Agency for Drugs and Technologies in Health (CADTH), which evaluates\u00a0and recommends\u00a0new drugs for insurance coverage within all federal, provincial and territorial public drug plans, except those in Quebec. CADTH\u00a0<a href=\"https:\/\/www.cadth.ca\/about-cadth\/who-we-are\">describes<\/a>\u00a0itself as \u201can independent, not-for-profit organization responsible for providing Canada\u2019s health\u00a0care decision-makers with objective evidence to help make informed decisions about the optimal use of drugs.\u201d\u00a0These decisions frequently lead to Canadians being denied\u00a0access\u00a0to\u00a0potentially life-saving or life-transforming\u00a0innovative medicines.\u00a0<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p>Why?\u00a0One reason, we argue,\u00a0is that\u00a0CADTH\u2019s drug review processes do not adhere to good\u00a0governance\u00a0principles, as we explored in a\u00a0<a href=\"https:\/\/www.dovepress.com\/articles.php?article_id=35696\">recent article<\/a>\u00a0in the peer-reviewed journal\u00a0<i>ClinicoEconomics\u00a0and Outcomes Research<\/i>. Governance is something\u00a0few\u00a0Canadians think\u00a0much\u00a0about.\u00a0But\u00a0decisions made using\u00a0processes that fail to follow good governance principles can negatively\u00a0impact\u00a0Canadians\u2019\u00a0lives.\u00a0<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">CADTH has separate processes for\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">the\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">review of cancer drugs and all other drugs<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">;<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">i<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">ts<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">\u00a0reviews of\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">noncancer\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">medications<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">result<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">being recommended for coverage<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">by public plans<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">.<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">The\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">outcome<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">\u00a0is that<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"SpellingError SCXW133892642\">Orkambi<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">\u00a0for cystic fibrosis<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">.\u00a0<\/span><span class=\"SpellingError SCXW133892642\">Orkambi<\/span><span class=\"NormalTextRun SCXW133892642\">\u00a0received a priority regulatory review\u00a0<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">(<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">meaning Health Canada considered that the drug filled an unmet need<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW133892642\">)<\/span><\/span><span class=\"TextRun SCXW133892642\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun They\u00a0are not\u00a0open to patients, pharmaceutical manufacturers, the public or the media,\u00a0and\u00a0their\u00a0proceedings are\u00a0not publicly available.\u00a0Patient groups and individuals\u00a0can\u00a0submit written comments but are not allowed to comment on draft reports\u00a0(other than\u00a0reports on\u00a0cancer drugs),\u00a0although drug manufacturers can.<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p>Participation\u00a0of\u00a0everyone\u00a0involved in decision-making processes\u00a0is important\u00a0if these\u00a0processes are to be credible and citizens\u00a0are not to feel\u00a0excluded.\u00a0The cancer drug\u00a0committee\u00a0includes\u00a0three patient representatives,\u00a0but\u00a0the other\u00a0committee\u00a0has none.\u00a0Patients are denied\u00a0genuine\u00a0participation in CADTH\u2019s drug recommendation processes, except with respect to cancer drugs.<span data-ccp-props=\"{}\">\u00a0<\/span>The absence of real accountability, transparency and participation in\u00a0CADTH\u2019s\u00a0drug recommendation processes means that opportunities do not exist for\u00a0everyone\u00a0involved to improve or maintain their well-being\u00a0or for reaching\u00a0true consensus.\u00a0Accountability and transparency are fundamental principles of good governance,\u00a0and\u00a0all\u00a0participants,\u00a0particularly\u00a0patients,\u00a0should\u00a0have an opportunity to improve their well-being by\u00a0being engaged\u00a0throughout the process and having a voice in decision-making.\u00a0<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p>The principles of good governance should be incorporated into reimbursement recommendation processes\u00a0but aren\u2019t,\u00a0because\u00a0CADTH has become\u00a0a willing partner\u00a0of federal, provincial and territorial governments\u2019\u00a0initiatives\u00a0to\u00a0contain\u00a0new drug costs.\u00a0This\u00a0was plainly demonstrated in a\u00a0<a href=\"https:\/\/ojrd.biomedcentral.com\/track\/pdf\/10.1186\/s13023-017-0611-7?site=ojrd.biomedcentral.com\">recent analysis<\/a>\u00a0of CADTH\u2019s recommendations for medications\u00a0for rare disorders. The positive recommendation rate for\u00a0rare disorder\u00a0drugs\u00a0with\u00a0an estimated annual cost of more than $55,000 was only 26\u00a0percent, whereas the rate for\u00a0those\u00a0costing less than $55,000 was 83\u00a0percent.<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-ccp-props=\"{}\"><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">The<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">\u00a0<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">current\u00a0<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">federal government was elected\u00a0<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">in 2015\u00a0<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">on a platform of openness,\u00a0<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">accountability,\u00a0<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">transparency and fairness<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">.<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">\u00a0<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">The external reviewers of the pan-Canadian health organizations\u00a0<\/span><\/span><span class=\"TextRun SCXW74413371\" lang=\"EN-CA\" xml_lang=\"EN-CA\"><span class=\"NormalTextRun SCXW74413371\">should<\/span><\/span><span class=\"TextRun
